Qualifying for a Discounted Fare ID Card

  • Customers with disabilities will need their physician to complete the appropriate sections of the Discounted Fare application. This form must be completed by a licensed physician (MD) or (DO). Applications completed by a Physician Assistant (PA), Registered Nurse (RN), or Nurse Practitioner (NP) will not be accepted. Once completed, bring the completed application and a State/Government/Military ID to one of the listed processing locations.
  • Seniors (65 years or older) do not have to complete the application. Simply bring a State/Government/Military ID to a listed location.
  • Medicare card holders do not have to complete the application. Simply bring a Government-issued Medicare card and a State/Government/Military ID to a listed location.

Getting your Discounted Fare ID Card

  • Discounted Fare IDs are processed at selected locations.
  • There is no fee to process and create the ID badge.
  • There is a $15 fee for replacement ID badges.
  • There is a limit of one (1) replacement ID badge per calendar year. You must bring your qualifying documentation in order to receive a replacement.
